In requesting the plumbing, drain cleaning, excavation or other services indicated, I understand that the condition of the pipes and lines cannot be readily determined and they may be deteriorated by causes such as age or exposure to chemicals. Therefore, in consideration of the services requested, I hereby, (a) Release and discharge Remarkable Drain Cleaning SVC Company, Owner, Affiliates, Officers, Employees, Agents, and Contractors collectively referred to as ("Remarkable Drain Cleaning SVC Light Industrial Service") from any claim for personal injury, property damage, or other damage, including consequential damages in connection with those services, (b) Agree to hold harmless and indemnify Remarkable Drain Cleaning SVC Light Industrial Service against any such claim made by a third party. This release and hold harmless will not apply to the extent a claim is caused solely by Remarkable Drain Cleaning SVC Light Industrial Service's negligence.

I represent that I am the owner or duly authorized agent of the owner(s) of the premises, and accept full responsibility for this agreement.
